SUPPORT COORDINATION
Support coordination helps you to make the best use of your supports in plan. Support coordination is a capacity building support which helps you to:

ACHIEVE DISABILITY SERVICES PTY LTD trained mentors can assist building confidence to access the community with a never ending list of opportunities. Activities are chosen entirely by the NDIS participant, their representative / nominee, or support coordinator, and supported by our mentors.
ASSISTANCE WITH DAILY LIFE
Assistance with, or supervision of, personal tasks of daily life to develop skills of the participant to live as autonomously as possible.
ACHIEVE DISABILITY SERVICES PTY LTD support workers provide additional assistance around the house as required. Supports of this nature range between each participant and their requirements. Tasks assisted with can be general, or specific to cater for health care requirements.
SHORT TERM ACCOMMODATION (Inc. Respite)
Providing integrated support for self-care, accommodation, food and activities in a centre or group residence for short periods.
ACHIEVE DISABILITY SERVICES PTY LTD offers respite supports for those eligible, to try new things, develop independence and learn new skills outside of the home. Respite breaks are not only beneficial for NDIS participants, but also provide a well earned break from care - for the NDIS participants families and caregivers.
TRANSPORTATION
ACHIEVE DISABILITY SERVICES PTY LTD provides assistance with transport to work, to school, to the shops, appointments, therapy supports and general activities within the community.
Maintaining independence is one of our main missions at ACHIEVE DISABILITY SERVICES PTY LTD. Providing assistance with transport in the community, to appointments, activities, programs, or to complete every day living tasks - is another high quality service offered by our staff; promoting our mission.
HOUSE AND YARD MAINTENANCE
Assistance to perform essential house and / or yard activities
At ACHIEVE DISABILITY SERVICES PTY LTD we are privileged to have an experienced house and yard maintenance crew available. Our house and yard maintenance crew will ensure communication is open, honest and inclusive - to guarantee the required needs are fulfilled. Decisions will be made with you, and conducted to a high standard.
